She almost gave herself away if the doctors and interns in the hospital hallway there noticed Sarah talking to thin air while she was hallucinating Kyle walking and talking beside her.
I suspect that conversation occurred only in Sarah's head.
What was awesome about Pooka!Kyle? Um, everything.
I liked that he was a steadying and as someone else said a gentling influence on Sarah and he was younger and prettier than she was (Sarah being in the middle of a particularly haggard-making day), completing the reversal of the normal gender roles. It seemed to me that the episode tried to create a different look for JJ from either D&D or GTAT - in this episode he looked pretty much the same age as John, highlighting how far Sarah has come since meeting him.
I found it interesting that Kyle's voice-over appears to wake Sarah, urges her to get out of bed and flee but it's not until his image actually appears to her that Sarah wigs. But even then, she doesn't appear to actually lose track of reality - she's aware she's hallucinating and overtly resistant to the dreamworld Kyle takes her to while she's under anesthesia. "Good wound" seemed to strongly imply that Sarah to a greater or lesser extent is and has always been guided by Kyle's "voice."
I thought it was interesting that the relationship played as one of adoration and devotion - two ascetics in mutual worship - rather than overtly sexual. We know Sarah has hardly been chaste for the past 17 years and while it would have been nice to see Sarah and Kyle mack on each other, it would have obscured the fact that what Sarah longs for is the emotional connection she had with Kyle, who was the last person she was able to speak with honestly. Sex Sarah can get anywhere. But, as the last scene with Felicia demonstrates, there is virtually no one Sarah can talk to.
